ABSTRACT

We investigated the contamination,antimicrobial resistance and the virulence genes carriage of Salmonella spp. in duck slaughter chain.Suspected strains were isolated from slaughterhouse samples according to GB 4789.4-2010 and identi-fied by duplex PCR,and then the positive strains were used for serotype analysis.Subsequently,positive strains were tested against 10 different antimicrobial agents using Kirby-Bauer disk diffusion method,the results were determined on the basis of CLSI standard.Finally,9 virulence genes were detected among positive strains by PCR.The results showed that 9 9 Salmonella isolates were recovered from 343 samples and total isolation rate was 28.86%.The prevalence of Salmonella before slaughte-ring,at depilation stage,at evisceration stage,in duck meat and after slaughtering was 45.71%,22.68%,24.72%, 38.33% and 25.81%,respectively.Seven serotypes were de-tected and most of them were S.Indiana,S.Newlands,S. Anatum.The Salmonella isolates were most frequently re-sistant to nalidixic acid(91.92%),the resistance rates of tet-racycline (43.43%),ampicillin (42.42%),trimethoprim-sulfamethoxazole (34.34%),ciprofloxacin (29.29%),ceftri-axone (27.27%),gentamycin (24.24%),and kanamycin (22.22%)were at a medium level.The resistance rates of amoxicillin-clavulanic acid (9.09%)and minocycline (6.06%)were relatively low.The multi-drug resistance rate of Salmonella isolates,which was 47.47%,showed a high especially in S.Indi-ana,S.Typhi and S.Typhimurium.It was notable that the harboring rates of virulence gene spvR(94.95%),avrA (93.94%),ssaQ(90.91%),mgtC(87.88%),sopB(83.84%),bcfC(80.81%),siiD(77.78%)among Salmonella isolates were at high level,in contrast to the lower carriage rates of spvB(29.29%),spvC (11.11%).In summary,the results indica-ted that the duck slaughter chain was easily contaminated by Salmonella spp.with different serotypes,different antibiotic re-sistant patterns and high virulence genes harboring rate.Relevant slaughterhouse and departments should strengthen supervi-sion in sanitation and manage the use of antimicrobial agents,to guarantee food safety and public health.

ABSTRACT

<p><b>BACKGROUND</b>Thymectomy is an established treatment for myasthenia gravis (MG), and video-assisted thoracoscopic surgery (VATS) thymectomy has become an acceptable surgical procedure. This study aimed to compare the results of VATS thymectomy and open thymectomy and to identify the prognostic factors after thymectomy.</p><p><b>METHODS</b>The clinical data of 187 consecutive thymectomies performed between July 2000 and December 2009 were retrospectively reviewed; 75 open thymectomies and 112 VATS thymectomies. Clinical efficacy and variables influencing outcome were assessed by Kaplan-Meier survival curves and Cox proportional hazards regression analysis.</p><p><b>RESULTS</b>The operative blood loss in the VATS group was significantly less than that in the open group ((62.14 ± 55.43) ml vs. (137.87 ± 165.25) ml, P < 0.05). The postoperative crisis rate increased with the severity of preoperative MG and the prescription dose of anticholinesterase. Complete follow-up information of patients more than 12 months after the thymectomy was obtained on 151 cases, 89 cases from the VATS group and 62 cases from the open group, with a mean follow-up period of 59.3 months, range from 12 to 117 months. Complete stable remission (CSR) was the end point for evaluation of the treatment results. The overall five-year CSR rate was 57.5%. Two good prognostic factors were identified; preoperative prescription of anticholinesterase alone (P = 0.035) and non-thymomatous MG (P = 0.003). The five-year CSR rate of the ocular type of MG reached a high level of 67.4%.</p><p><b>CONCLUSIONS</b>Thymectomy can achieve good long-term CSR in MG, and VATS is an ideal alternative method. High-dose prescription of anticholinesterase and the advanced stage by Myasthenia Gravis Foundation of America (MGFA) classification have higher risks of postoperative crisis. Preoperative prescription of anticholinesterase alone and non-thymomatous MG are good prognostic factors. Thymectomy should also be considered for the ocular type of MG.</p>

ABSTRACT

Objective To observe the effects of early rehabilitation on the serum neuron specific enolase (NSE) levels of patients with acute cerebral infarction.MethodsSixty patients with acute cerebral infarction were randomly divided into a rehabilitation group and a control group. All received routine treatment at the acute stage, including anti-platelet aggregation medication, drugs for improving microcirculation, neurotrophic agents and prompt treatment of any complications. Patients in the rehabilitation group also received systemic rehabilitation training beginning immediately after their vital signs had been stabilized. NSE in serum was assayed before treatment and after 3, 7and 14 days. National Institutes of Health stroke scale (NIHSS) scores were evaluated at each time point, and the two groups were compared.ResultsThere was no significant difference in serum NSE or NIHSS scores between the two groups pre-treatment. Both groups improved to a certain extent, but the improvements in the rehabilitation group were significantly better than in the control group, as their NSE levels at 7 days and NIHSS scores at 14 days were both significantly better.ConclusionsEarly rehabilitation intervention contributes to reducing serum NSE levels after acute cerebral infarction, lessening brain injury, and thereby promoting the recovery of damaged neural function.That may be one of the mechanisms by which early rehabilitation promotes functional recovery in patients with acute cerebral infarction.

ABSTRACT

Objective: To develop and prepare oral rehydration salt powder II (ORS II) effervescent tablets. Methods: Taking the mass/tablet, rigidity, disintegration time, and pressing status as the main indicators, we optimized the formulation and preparation process of the effervescent tablet by single factor test. The character, identification, disintegration time, acidity, and content determination of the tablets were also investigated. Results: It was found that the optimized preparation process of the effervescent tablets was to mix the acid part, which was granulated with ethanol, with the base part, and then compressed the mixture. The optimal formula was composed of: NaCl (11.5%), KCl (4.93%), C6H8O7 (6.21%), NaHCO3 (8.15%), C6 H12 O6 (65.7%), and PEG-6000 (3%). The rigidity of the prepared effervescent tablet was 4.5 kg and the disintegration time was 175 s. Conclusion: The formulation of the present product is consistent with the corresponding Chinese Pharmacopoeia monograph, requiting no additional effervescent; the preparation process is simple and the products are stable, and the product my be developed into a new oral rehydration salt formulation.

ABSTRACT

Objective To investigate the relation between Fas-mediated apoptosis and glucocorticoid treatment in myasthenia gravis (MG). Methods In 17 patients with MG, 6 patients received glucocorticoid treatment (glucocorticoid treatment group),and 11 patients were treated without glucocorticoid (nonglucocorticoid treatment group). Meanwhile, 13 healthy cases were selected as healthy control group. CD4,CD8 and Fas expressions in peripheral blood T lymphocyte were detected by flow cytometry in three groups and analyzed. Results The percentage of CD4-CD8+ cells in peripheral blood T lymphocyte in glucocorticoid treatment group was significantly higher than that in healthy control group[(36.75 ± 11.56)% vs. (26.31 ±9.00)%, P = 0.027], while the percentage of CD4-CD8- cells was significantly lower [(30.56 ± 9.72)% vs.(42.96 ± 11.54)%, P =0.018]. The percentage of CD4-CD8+ cells in peripheral blood T lymphocyte in glucocorticoid treatment group was significantly higher than that in non-glucocorticoid treatment group [(36.75 ± 11.56)% vs. (25.24 ±7.63)% ,P =0.019]. The percentages of Fas+ and CD8 +Fas+ cells in peripheral blood T lymphocyte in glucocorticoid treatment group were significantly higher than those in healthy control group[(46.10 ± 7.13)% vs. (31.22 ± 13.00)%, P=0.006; (62.86 ± 12.29)% vs. (45.59 ±11.50)%, P = 0.003]. The percentage of CD8+ Fas+ cells in peripheral blood T lymphocyte in glucocorticoid treatment group was significantly higher than that in non-glucocorticoid treatment group [(62.86 ± 12.29)%vs (50.84 ± 8.31 )%, P = 0.034]. Conclusions Glucocorticoid treatment may have influence on peripheral blood T lymphocyte subsets in patients with MG. Fas-mediated apoptosis may be involved in the mechanism of glucocorticoid treatment in MG.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the clinical effect and the influences of Shuxuetong injection on serum neuron-specific enolase (NSE) level, the neurological deficit and activities of daily living in patients with acute cerebral infarction.</p><p><b>METHOD</b>The 80 patients with acute cerebral infarction were randomly divided into Shuxuetong treatment group (40 cases) and routine control group (40 cases), both received routine treatment, while Shuxuetong injection was given additionally to treatment group. The serum NSE level, the National Insitute of Health Stroke Scale (NIHSS) scores and the clinical effect were observed pre-and post-treatment. The Barthel Index (BI) was evaluated after one month.</p><p><b>RESULT</b>The serum NSE level and NIHSS scores in two groups of post-treatment decreased obviously than those of pre-treatment, and after treatment in Shuxuetong treatment group the serum NSE level and NIHSS scores were significantly lower than those in control group, the differences were significant (P<0.05). Effective rate of Shuxuetong treatment group was 87.5%, and control group was 65%, the difference of the clinical effect between the two groups was significance (P<0.05). After one month BI of post-treatment in two groups improved than those of pre-treatment, and Shuxuetong treatment group was significantly better compared with control group (P<0.05).</p><p><b>CONCLUSION</b>Shuxuetong injection has the remarkable neuronal protective effect, can decrease the serum level of NSE after acute cerebral infarction, promote recovery of nerve function, reduce disability rate, and improve quality of life and prognosis of patients with acute cerebral infarction.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the learning curve of single-direction complete video-assisted thoracoscopic surgery (cVATS) for lung cancer.</p><p><b>METHODS</b>From May 2006 to April 2009, 125 cases of cVATS for lung cancer were performed by two dedicated surgeons. Clinical data were collected prospectively and analyzed retrospectively. The patients operated by different surgeon were divided into 2 groups (group A, n = 24; group B, n = 101), and group B was further divided sequentially into 4 subgroups (B1, B2, B3 and B4) by the number of patients. The patients in group A and B were operated by the surgeons with 2-year and 5-year experience of VATS respectively. The operating time, blood loss, number of resected lymph nodes (NLN), rate of thoracotomy conversion (RTC) and postoperative complications (POC) were compared.</p><p><b>RESULTS</b>Compared with group B, the operating time of group A was significantly prolonged [(237 ± 85) min vs. (187 ± 43) min, P = 0.013], but there were no significant differences in blood loss, NLN, RTC and POC. Comparing group A with B1, the same results were got. From group B1 to B4, the operating time was gradually reduced and blood loss decreased, but the difference was not statistically significant. And in group B, there was a significant reduction of blood loss for the last 51 cases compared to the first 50 cases [(122 ± 141) ml vs. (87 ± 81) ml, P = 0.009].</p><p><b>CONCLUSIONS</b>At the early stage of cVATS resection of lung cancer, the duration of operation was longer, which it was more significant for the surgeons with short carrier of thoracoscopic experience. But the morbidity of operation related complications did not increase. The indicator of proficiency in this operation is achievement of 50 cases of complete thoracoscopic resection of lung cancer.</p>

ABSTRACT

<p><b>OBJECTIVES</b>To observe the clinical outcome of invasive thymoma, and analyze how the surgical methods, Masaoka staging, adjuvant radiotherapy and/or chemotherapy affect the prognosis.</p><p><b>METHODS</b>The clinical data of 59 surgical patients with invasive thymoma and conducted follow-up from January 2000 to December 2009 was analyzed retrospectively. There were 34 male and 25 female, aged from 18 to 72 years with a mean age of 49 years. Forty-four cases underwent radical resection while the other 15 cases underwent palliative resection or biopsy. Masaoka staging: 18 cases with stage II, 30 cases with stage III, 11 cases with stage IV. Patients with stage II didn't undergo further adjuvant radiotherapy or chemotherapy after surgery. Among the patients with stage III and stage IV, 26 patients received adjuvant radiotherapy and/or chemotherapy after surgery, while the other 15 patients did not receive any further therapy. The relationship between the prognosis and the different surgical methods, Masaoka staging, adjuvant radiotherapy and or chemotherapy was evaluated.</p><p><b>RESULTS</b>Fifty-nine patients had been followed up for 1 to 111 months with an average of 54 months. Three cases were lost with the rate of 6.1%. Nineteen patients suffered local recurrence or systemic metastasis, and 14 of them died. The 3-year and 5-year survival rates were 86.8% and 70.8% respectively. Univariate analysis indicated that patients with early Masaoka staging and who received radical resection, adjuvant radiotherapy and/or chemotherapy after surgery had better survival (P < 0.05). Multivariate analysis indicated that radical resection, adjuvant radiotherapy and or chemotherapy were the most significant prognostic factors which could remarkably improve the survival of patients (P < 0.05). For patients with resectable recurrence, reoperation could also improve survival.</p><p><b>CONCLUSIONS</b>The Masaoka staging is related to the prognosis of patients with invasive thymoma. Radical resection, adjuvant radiotherapy, chemotherapy can significantly improve the survival of patients with invasive thymoma. Reoperation can improve the survival of some patients with recurrence.</p>

ABSTRACT

Objective To evaluate the relationship between post-stroke urinary incontinence(UI)and 1 year stroke outcomes.Methods One hundred and thirty consecutive cerebral stroke survivors were classified into 2 groups according to their incontinence status at 1 to 10 d after onset.The patients'baseline characteristics,1 year mortality rate,abilities in the activities of daily living(ADL)and Modified Rankin Scale(mRS)scores were compared.Multivariate analysis was applied to highlight UI risk factors and correlations between UI and 1 year outcomes.Results Total anterior circulation syndrome(TACS)numbers and National Institutes of Health stroke assessment (NIHSS)scores in the UI group exceeded significantly those in the non-UI group,and both were significantly associated with initial UI.The 1 year mortality rate,patients'ADL dependence and mRS scores in the UI group exceeded those in the non-UI group significantly.Initial UI was also significantly associated with the 1 year outcome independently,as well as with age>75 years,Oxfordshire Community Stroke Project(OCSP)classification and initial NIHSS score.Conclusion UI is common in the patients with TACS and higher initial NIHSS scores.Initial UI predicts a poor 1 year outcome,SO treatment should be strengthened.

ABSTRACT

The phyA(m) gene encoding acid phytase and optimized neutral phytase phyCs gene were inserted into expression vector pPIC9K in correct orientation and transformed into Pichia pastoris in order to expand the pH profile of phytase and decrease the cost of production. The fusion phytase phyA(m)-phyCs gene was successfully overexpressed in P. pastoris as an active and extracellular phytase. The yield of total extracellular fusion phytase activity is (25.4+/-0.53) U/ml at the flask scale and (159.1+/-2.92) U/ml for high cell-density fermentation, respectively. Purified fusion phytase exhibits an optimal temperature at 55 degrees C and an optimal pH at 5.5~6.0 and its relative activity remains at a relatively high level of above 70% in the range of pH 2.0 to 7.0. About 51% to 63% of its original activity remains after incubation at 75 degrees C to 95 degrees C for 10 min. Due to heavy glycosylation, the expressed fusion phytase shows a broad and diffuse band in SDS-PAGE (sodium dodecyl sulfate-polyacrylamide gel electrophoresis). After deglycosylation by endoglycosidase H (EndoH(f)), the enzyme has an apparent molecular size of 95 kDa. The characterization of the fusion phytase was compared with those of phyCs and phyA(m).

ABSTRACT

<p><b>OBJECTIVE</b>To assess the metastatic frequency in different groups of lymph nodes and its influencing factors of the thoracic esophageal squamous cell carcinoma (ESCC) in order to determine the extent of lymphadenectomy during esophagectomy.</p><p><b>METHODS</b>The clinical data of 730 patients with ESCC who underwent esophagectomy and lymphadenectomy were analyzed retrospectively.</p><p><b>RESULTS</b>Of 730 patients, 166 had metastasis to the para-esophageal lymph nodes (22.7%), 90 to the left gastric artery lymph nodes (12.3%), 67 to the lymph nodes around gastric cardia, and 15 to the subcrinal lymph nodes (2.1%). Univariate analysis showed that metastasis to the subcrinal lymph node was positively correlated with the length and differentiation of tumor (P < 0.05), but it was not correlated with any the above parameters when analyzed by multivariate analysis. The metastasis to the para-esophageal lymph node was positively correlated with the length, invasion depth and differentiation of tumor by univariate and multivariate analysis (P < 0.05). The metastasis to the lymph nodes around gastric cardia and metastasis to left gastric artery lymph nodes were positively correlated with the position and invasion depth of tumor by univariate and multivariate analysis (P < 0.05).</p><p><b>CONCLUSION</b>Lymph nodes of the para-esophagus, gastric cardia and left gastric artery usually have high frequency to harber mestastasis, therefore, it was suggested that the lymph nodes in these groups should be dissected during esophagectormy with two-field lymphadenectomy for thoracic esophageal squamous cell carcinoma. Whereas for those patients with the lesion < 3 cm in length or with tumor invasion confined within the esophageal wall or with a lesion located at the upper or lower third of the thoracic esophagus, the subcrinal lymph nodes may not be necessarily dissected.</p>

ABSTRACT

Objective To investigate the dynamic change and different apoptosis of T lymphocyte subsets in patients with acute pancreatitis.Methods Forty-four patients with MAP and 32 with SAP were enrolled in this study.Blood samples were collected on the lst,3rd and 7th d after onset.T lymphocyte subsets and their apoptotic rate were determined by flow cytometry.Results Compared with the control group and MAP group,CD4+ T lymphocytes and the ratio of CD4+/CD8+remarkably decreased while the apoptotic rate of CD4+ T cells significantly increased in patients with SAP.However,there was no marked difference in apoptotic rate of CD8+ T cells among different groups.Conclusion The different apoptosis of T lymphocyte subsets in AP,especially in SAP,results in severe and sustained cellular immune suppression.

Objective To study the expression of eytokines in acute hemorrhage patients with systenlic in-flammatory response syndrome(SIRS)and multiple organ dysfunction syndrome(MODS).Methods Platelet acti-vation markers CD62p and cytokines in acute hemorrhage patients with SIRS and MODS were detected by using flow cytometry,and compared with that of control group.Results The expression of CD62p and cytokines(TNF-α and IL-6)in acute hemorrhage patients,with or without SIRS and MODS wag significantly higher than that in control group(P<0.01),and higher in MODS group than in SIRS group,and in MODS death group than in MODS survival group(P<0.01).Conclusion The activation of platelet and the over-expression of cytokines participate in the on-set and development of acute hemorrhage,SIRS and MODS,and were related with the severity of disease and progno-sis.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the expression of Toll-like receptors (TLRs) in thymus of myasthenia gravis (MG) patients and the relationship with clinical features.</p><p><b>METHODS</b>Thymic specimens of 36 patients received extended thymectomy for MG were divided into three groups by pathological type: 13 thymoma tissues (thymoma group) and 13 thymic tissues adjacent to thymomas (parathymoma group) from 13 cases of MG patients with thymomas, and 23 thymic tissues from MG patients without thymomas (MG nonthymoma group). Twenty-one normal thymic specimens from cardiac surgery were used as controls. The levels of TLR2-4 mRNA were examined by RT-PCR, then the levels of TLR4 mRNA were assayed by real time RT-PCR and their relationship with clinical features were analyzed.</p><p><b>RESULTS</b>The levels of TLR4 mRNA among the different groups had significant differences, while there was no difference in TLR2 and TLR3 levels. The real time RT-PCR showed that the level of TLR4 mRNA in nonthymoma group was significantly higher than that in control group(0.8544+/- 0.1200 vs 0.6851+/- 0.1524, P=0.018). And so is parathymoma group compared with the thymoma group (0.8214+/- 0.1019 vs 0.7101+/- 0.0916, P=0.005). No significant difference of TLR4 mRNA level was found between the parathymoma and nonthymoma groups. Nevertheless, the expression of TLR4 in both groups was increased compared with control group. The levels of TLR4 mRNA had positive correlation with Osserman type(R=0.609; P=0.004) .</p><p><b>CONCLUSION</b>TLR4 may play a key role in the pathogenesis of MG. It was the thymic tissues adjacent to thymomas but not thymomas themselves participated in the onset of MG.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the feasibility of detecting p53 gene mutation in exfoliative esophageal cells, and compare gene mutation between precancerous lesions and normal esophageal exfoliative cells and correlate p53 gene mutation with esophageal carcinogenesis.</p><p><b>METHODS</b>Forty-eight samples (24 normal squamous epithelia and 24 severe squamous dysplasia) were obtained by balloon cytologic technique from a high incidence area, Yanting county, Sichuan Province, China in 1982. p53 gene mutations in exons 5 and 7 were analyzed by PCR-SSCP.</p><p><b>RESULTS</b>p53 genes were detected in all samples. Five samples with p53 mutation were detected in exon 7 and no mutation was detected in exon 5 in 24 severe dysplasia samples. Three of the 5 samples with mutation in exon 7 developed esophageal cancer in 1992, 1994 and 1996 respectively. No p53 gene mutation was detected in exon 5 and 7 in normal exfoliative samples.</p><p><b>CONCLUSION</b>p53 mutation may have occurred in the precancerous lesions which contributes in the initiation of human esophageal carcinogenesis.</p>
